In the last 13 years Streamline’s software and behavioral health team has worked with many health care organizations to develop customize software app that improve the quality and efficiently of health care.





TYPE OF ORGANIZATION:
Province of BC Gaming Ministry.

SCOPE OF WORK: MyOutcomes was requested to conduct a business analysis to uncover the key workflow processes for 3 departments within the Province of BC Gaming Ministry. Technical specifications were developed and the software development was completed based on specifications. These technical specifications would lead to the development of an enterprise system that would cross all 3 departments. The project continues to be implemented with an end goal to have MyOutcomes at the centre of their Clinical division.

OUTCOME: MyOutcomes completed the Business Analysis for all 3 Departments. Also – process maps and business requirements documents were complete for the main aspects of the enterprise system. MyOutcomes continue to maintain the MyOutcomes system and is continually improving the workflow processes to create operational efficiencies.

TYPE OF ORGANIZATION:
The leadership of the Alberta Government, Calgary West Central Primary Care Network were seeking ways to create preventative initiatives to enhance patient care in daily Doctor visits.

SCOPE OF WORK: Streamline published a series of 40 wallet card that fit into a plastic wallet card rack in examination room for use of over 400 Doctors. The Doctor would select a wallet card that was relevant to supporting the patient’s specific needs. The card had a summary recommendation towards patient self-care initiatives and lead the patient to a large porthole where additional interactive tools and videos were provided.

OUTCOME: This process was used for over 2 years successfully, but no longer exists due to Goggle search engines that emerged as an even superior method of searching for patient self-care initiatives.

TYPE OF ORGANIZATION:
Used by over 56 organization in Canada

SCOPE OF WORK: A Health Risk appraisal (HRA) was required to measure employee population health to determine if wellness programs were effective and producing return on Investment. Streamline gained access to the Framingham, Diabetes association and Heart and Stroke Foundation algorithms and produced the HRA programed in .NET and with MySql data base. User would register and be reminded to do the HRA every year showing comparatives. Additionally where individual scores were low, recommendation were created to support new self care initiatives.

OUTCOME: The Streamline Health Risk Apprasial was developed in 2010 and is still being used in various organizations to measure population health and produce confidential and individual and aggregate reports while complying to Canadian and USA confidentiality acts.

Over 130 private and government organization used this wellness initiative to improve employee health.

SCOPE OF WORK: Streamline created a large multi-media interactive health portal that was used as a cloud based corporate health and wellness initiative to improve population health. Initiatives included wellness activities, assessment tools, web-n-r lunch and learn workshops, and telephone and email counselling. A validated health Risk Appraisal was programed using the Framingham and Diabetes Association data to creat annual corporate health and wellness report card used individuals and provided aggregate reports on total population health.

OUTCOME: This wellness initiative is still being used today and works to increase population health

TYPE OF ORGANIZATION:
An online tool that helps counselors and therapists improve effectiveness by way of outcome measuring and e-learning.

SCOPE OF WORK: Streamline developed algorithms that help therapist track each client session results. This data is fed through algorithms which provide meaningful results to the clinician and supervisors. Additional e-learning modules were provided to help therapist improve skills in areas reported.

OUTCOME: Myoutcomes was the recipient of the 2015 UK Mental Health innovation of the year award. Over the past 5 years Myoutcomes tools are used by over 3000 therapists in large and small government and private mental health agencies in over 20 countries including the USA military and John Hopkins University.

TYPE OF ORGANIZATION:
Automotive warranty Insurance providers with dealer outlets across Canada.

SCOPE OF WORK: Streamline converted a paper base system to online data warehousing. An internal and external server was built and installed and linked to a web based application for dealer to access all services, products, contracts and claims.

OUTCOME: Significant efficiency and data security was achieved. Miscellaneous HR services were also provided over the 9 year life span of this client. Streamline continues to provide maintenance and upgrades to this client.

TYPE OF ORGANIZATION:
Interactive online employee corporate wellness program.

SCOPE OF WORK: Built a 20,000 page interactive health portal with a personal user data-base to track individual health improvements. The portal was compliment with USA (HIPAA) and Canadian (PIPEDA) confidentiality acts which included tier three level servers and intrusion technology firewalls.

OUTCOME: 60k thousand employees from various organization had access to online tools, team challenges, stress management workshops, tele – counselling and Health Risk appraisals with reported health improvements to 23% of users.

TYPE OF ORGANIZATION:
Enterprise software for hospital operations.

SCOPE OF WORK: Developed a Human Resource program with full implementation.

OUTCOME: Increased employee retention and team commodore and reduced legal claims.

TYPE OF ORGANIZATION:
On line mental health prevention and recovery services.

SCOPE OF WORK: A group of mental health professionals had a vision to help people build resilience to ward off stress and mental illness using a web based application integrated with and telephony technology. Stream line worked with this team to develop a project spec. Upon completion of the spec a data base, auto sign on, smart app and interactive videos were developed to engage users with therapist and participants.

OUTCOME: This product was being beta tested in various mental health agencies and within private business HR departments. Health Master is also a strategic partner with the Canadian mental health association.
